Therapy Placement Application

The application will take approximately 5-10 minutes to complete. Progress cannot be saved, so please ensure you have the following information and documents ready before you begin:

• DBS certificate number
• Professional liability insurance policy number
• Student membership number (i.e. BACP, etc)
• Qualification/s certificate/s
